Public Health Model & Organizational Setting/Support
During the third week of NUR4636L, you will select a Behavioral Health, Public Health, or Change Model to serve as the underpinning for your project. You will post this as your initial post, receive suggestions and questions from faculty and student colleagues, and then, once you receive the detailed feedback from your faculty member, make all necessary changes/edits and cut/paste the content of this worksheet into the COMMUNITY Project. Remember to include references on a separate page and maintain APA formatting.
Theoretical Model/Framework
Identify the theory or model that will serve as the framework for your project. We reviewed at least one public health theory (Health Belief Model [HBM]) in NUR4636. This may be a translational science theory, public health-centric nursing theory or a theory from the discipline of public health, such as the Health Belief Model. As you introduce the theory, identify the main tenets of the model, then explain how the model helps inform the steps in your evidence-based education intervention. Begin with a description of your selected change model and why it was selected as the model for this specific change. You should learn about the model from a primary source (i.e., do not cite a textbook). The model you select should be appropriate to the change you recommend.
Organizational Setting
This section should describe the setting for the evidence-based education intervention. Include the type of setting, a description of a typical “client”, and approximately how many community members may be reached through the intervention you’ve selected. How will you “find” your audience? Will you educate one person at a time or many at once? Do you need permission to talk with the community members you’ve selected? Education might be appropriate in schools, churches, clubs, sports teams, non-profit organizations, homeless shelters, halfway houses, neighborhood groups, etc. The local health department may also have ideas. You MAY NOT target acute care facilities/patients for your intervention. It must be community-facing. Do you need written permission to be present or schedule a virtual discussion? If so, who are the decision makers/stakeholders? Do they need to hear from CFK as well? Keep in mind, this is NOT a clinical, and at no time will you be delivering direct patient care.
